The Critical First 6 Weeks After Total Knee Replacement

Undergoing Total Knee Replacement (TKR) surgery is only 50% of your recovery journey — the remaining 50% depends entirely on structured, daily postoperative physical therapy. Many patients in Hyderabad struggle with post-surgical fear, knee stiffness, and severe swelling during the first few weeks.

Traveling to a physical therapy clinic in a car or auto during the early post-op phase causes unnecessary knee trauma and increases the risk of wound dehiscence. In-home postoperative rehabilitation ensures ICU-level care in your own bed, where your knee can be safely mobilized without travel stress.

Key Phases of At-Home TKR Rehabilitation

Phase 1: Infection Control & Swelling Reduction (Days 1 to 14)

Our priority during the first two weeks is achieving full knee extension (straightening the leg to 0°) and keeping surgical swelling under control.

  • Ankle Pumps & Quad Sets: Firing the quadriceps muscle to prevent postoperative muscle inhibition and deep vein thrombosis (DVT).
  • Cryotherapy (Cold Therapy): Applying ice compresses for 15-20 minutes every 3 hours to drain fluid.
  • Passive Range of Motion (PROM): Gentle, manual knee bending guided by your physiotherapist to reach 90° flexion safely.

Phase 2: Gait Re-Education & Knee Bending (Weeks 3 to 6)

During this stage, we transition the patient from a walker to a elbow crutch or walking stick while increasing knee bending toward 120°.

  • Heel Slides: Sliding the heel along the bed sheets while gently pulling the knee toward the chest.
  • Straight Leg Raises (SLR): Strengthening the quadriceps to lift the full weight of the leg without lag.
  • Stair Climbing Protocol: Teaching the clinical rule: "Up with the good leg, down with the bad leg."

Achieving 120° Bending Without Surgery Revision

If knee flexion remains under 90° past 6 weeks, scar tissue (arthrofibrosis) can lock the joint. Our physiotherapists apply gentle joint mobilization (Maitland techniques) and patellar glides to prevent joint capsule tightness.
