What is Frozen Shoulder (Adhesive Capsulitis)?

Frozen Shoulder, clinically termed Adhesive Capsulitis, is a debilitating condition characterized by severe pain, stiffness, and profound loss of active and passive shoulder movement. The connective tissue capsule surrounding the glenohumeral joint becomes inflamed, thickens, and tightens, severely restricting the ball-and-socket mechanism.

In Hyderabad, frozen shoulder is especially prevalent among individuals aged 40 to 65 and is up to 5 times more common in patients with diabetes or thyroid conditions.

The 3 Clinical Stages of Frozen Shoulder

Adhesive Capsulitis classically progresses through three distinct clinical phases, each requiring a tailored physiotherapy strategy:

Stage 1: The Freezing Phase (2 to 9 Months)

Pain is the dominant symptom. Any shoulder movement causes intense sharp pain, particularly at night. Range of motion begins to decrease. Physiotherapy Focus: Gentle pain-relieving modalities (IFT, low-grade Maitland Grade I/II glides), cryotherapy, and pendulum exercises to prevent rapid capsule thickening without triggering inflammation.

Stage 2: The Frozen Phase (4 to 12 Months)

Pain may slowly plateau or diminish, but stiffness becomes extreme. Reaching behind the back (internal rotation) or reaching overhead becomes almost impossible. Physiotherapy Focus: Intensive end-range Maitland Grade III/IV joint mobilizations, posterior capsular stretching, and targeted dry needling of hypertonic rotator cuff muscles.

Stage 3: The Thawing Phase (12 to 24 Months)

The joint capsule gradually relaxes. Physiotherapy Focus: Progressive strengthening of the rotator cuff, scapular stabilizers, and functional overhead loading to return to sport and daily lifting activities.

Why Manual Therapy Mobilization is Superior to Waiting

Without active physiotherapy, frozen shoulder can take up to 3 years to resolve, often leaving behind a 15-20% permanent loss of overhead reaching. Advanced manual therapy (Maitland and Mulligan mobilizations) significantly shortens recovery time to 8-12 weeks.

Our senior orthopedic physiotherapists at Athletlance specialize in:

  • Inferior & Posterior Glenohumeral Glides: Specific directional forces that stretch the tightest folds of the joint capsule.
  • Dry Needling & Myofascial Release: Deactivating trigger points in the subscapularis, infraspinatus, and trapezius.
  • Scapulothoracic Rhythm Retraining: Ensuring the shoulder blade moves in sync with the arm to prevent secondary neck and upper back pain.
