The Challenge of Knee Ligament Injuries

An Anterior Cruciate Ligament (ACL) tear is one of the most devastating injuries an athlete can face. Whether you are a weekend football player in Hyderabad or a competitive badminton player, restoring knee stability is critical. A poorly rehabilitated knee leads to early osteoarthritis and a high risk of re-rupture.

The Evidence-Based ACL Rehabilitation Phases

At Athletlance, we follow an advanced, phase-based recovery protocol derived from sports science standards. Each phase must be cleared using objective strength criteria, not just timeline checklists.

Phase 1: Pain Control & Knee Extension (Weeks 0-2)

The primary goal immediately after surgery or injury is to eliminate swelling and achieve full, passive knee extension (straightening). If a patient cannot straighten their knee in the first 2 weeks, scar tissue can permanently limit movement.

  • Patellar mobilizations to keep the kneecap moving freely.
  • Quad sets and straight leg raises to prevent muscle shutdown.

Phase 2: Progressive Loading & Quad Strength (Weeks 3-8)

Once swelling is minimal and the patient can walk without a limp, we focus on rebuilding the quadriceps and hamstring muscles.

  • Double and single-leg squats, lunges, and leg presses.
  • Bilateral and unilateral calf raises.
  • Stationary cycling to improve joint lubrication and cardio endurance.

Phase 3: Proprioception & Balance (Weeks 9-16)

A damaged ligament loses its nerve sensors (proprioceptors), which tell the brain where the joint is in space. We retrain these sensors to prevent the knee from 'giving way'.

  • Single-leg balance drills on unstable surfaces (like Bosu balls).
  • Perturbation training (gentle pushes while balancing).

Phase 4: Agility, Plyometrics & Return-to-Sport (Months 5+)

This is where standard physiotherapy fails and sports-specific training is required. Before clearing an athlete, we perform a Limb Symmetry Index (LSI) test. The injured leg must have at least 90% of the strength and landing capacity of the healthy leg.

  • Deceleration training (learning how to stop and cut safely).
  • Double-leg and single-leg vertical jumps.
  • Sport-specific drills (shuttle runs, change of direction).

Answers to Your Questions

Questions Answered In This Article

Can an ACL tear heal without surgery?

Partial ACL tears and some full tears in non-athletes can be managed non-surgically using advanced sports physiotherapy to strengthen the surrounding quadriceps and hamstrings.

How long does ACL rehab take?

A complete return to competitive cutting and contact sports typically takes 6 to 9 months of structured rehabilitation to ensure the ligament graft is fully mature.

What is Limb Symmetry Index (LSI)?

LSI is a clinical measurement comparing the muscle strength and hop distance of the injured limb to the healthy limb. It must be above 90% before returning to sports.
